I put part A of all applicants together with 1 common part B as 1 bundle. Then I repeated this. The rest of the docs were separate bundles (separate for each applicant). Note that I did not make any copies of part A or B. I actually filled them twice.

USIndus, u still have to apply for SC first. The SC app form has a section for lost passport case as well.

I applied for SC and got my cancelled Indian passport back. I sent it again with my OCI app last week, not sure if I'll get it back....

its a good Q....I was also wondering this before I sent my SC app...the issue is that SF CGI site clearly mentions that we need SC done before submitting OCI app, so I just followed it. I sent SC app on Aug31, got the SC on Oct3 and sent OCI app 2 days back.

Seeker, Please see list of docs on OCI website. For minors, it asks for birth cert AND copy of US passport.
