Yellow Form Questions

Could anyon help me with the following questions regarding the response to the Yellow Form:

1) What should be included in the Employment Letter? It should be a long letter? Where can I possibly get a sample letter?

2) H-1: is it enought if I include the H-1 approval notice? What else I should submit regarding my H-1?

3) I have EAD but have not worked on it. Do I need to submit a copy of my EAD?

(1) Employment letter should state the followings:
   1. Job duty that you list in I-140 application
   2. Company intents to hire you forever
   3. You will not get paid less than the salary stated on your LC
(2) Yellow form requires H1 info?
(3) You still need to send the copy of EAD even if you never used it.
