Wrong dates on GC: Gurus please advise

My wife got here green card stamped on the passport on April 2, 2001. I also have the I-485 which says the sent date for the I-485 was March 20th, 2001.
However her GC says her PR starts Aug, 1998 and expires Aug, 2011.
As per the 3 month rule, she is eligible to apply for Naturalization.
What do I put on the N-400 form as "Date she become a Permanent Resident" ? 1998 might look odd, so I am thinking April 2, 2001 ? Please advise
Is it advisable to send a copy of the passport stamp and a copy of the I-485 with a note explaining all of this on the copy of the GC.
I have 2 other brief questions:
1. Is sending the application by USPS Express - no signature confirm , OK ?
2. The fees for the application - do we have to combine the USCIS and FBI fee in one check or are these going to be seperate checks ?

I am not aware of "FBI" fee. The fees are the application fee and finger print fee. you can put them all in 1 check.
