My husband received his plastic which has incorrect country of birth. The local office (Boston) doesn't take his case. He has to file it with VSC (since it was issued by them.) When he called the IIO, it was confirmed that mistake was done by VSC (not us since we didn't put the wrong country of birth in our application). IIO told my husband to mail the card directly to them. I don't think he was told to submit any forms ( I might be wrong since it was 1 year ago...) But we did attach a note to let them know about this problem. 3 months later, no news yet from VSC. My husband called and was informed that he should put the IIO's name on the envelope. Now he has to wait for another 3 months because his mail was 'probably' in the 'general mail' room (in other words, lost in that big pile of mails until someone sorted it out). There was no point in questioning them why that IIO didn't tell us that information even though my husband specifically asked him if he should put 'any' dept name on it. It took 6 months for my husband.
So you might want to contact the local office first and if they could take your case, that's better. If not, contact the service center and get all the information before sending your card to them.
