Would like to use AC21, what do I need?

I would like to use AC21, even though I am aware that it is optional.

Case is like this,

1. Got laid off from sponsoring company (A) on April 2003.
2. Joined another company (B) in May 2003.
3. Left B and joined company C in Nov 2003 for better pay.
4. Left C and joined company D in Jan 2004 for better pay.

Company D is a permamnet job while company C contracted me to work for company D.

All these jobs were same at companies A,B,C and D. The location is also same.

What documents do I need to inform USCIS regarding AC21?

Please help.

I would like to send the following documents,

1. A cover letter stating the use of AC21, the law, its section # etc. and my filing dates as well as eligibility. ANy format is welcome.

2. Copy of AC21 memo published in Aug 2003.

3. Company letter (EVL), indicating salary, title, date of employment, nature of employment (fulltime/40 hr per wk), intent to hire as a permanent employee, and job description (same or similar to the one in LC)

4. Latest pay stubs.

5. Latest W2s.


Please correct me where I am wrong and please feel free to suggest me any additional document that I would need.

Gurus, please help me in getting the right information in AC21 packet.
