Wifes change of address

My wife enetred US on green card 2 months back. We got the plastic for her with her maiden name,

Now we want to change last name.
I underdstand for name change.
- Need to go Indian Consulate
- SSN office

How do I get the new GC plastic card???? What do I need to do??
