Why are people\'s I140 AD and I485 ND so far apart ?

Guys,

Why are people getting the I140 AD in say March and then the I485 ND
is in say June. Don\'t you have to just get your medical exam and
then green card photos and that\'s it.

Right ?

Am I missing something you need to do.
