Which employer should I fill in: the consulting company or the acutal company I'm doing cntract work

If I'm hired as a contract worker by consulting company A, and actually working the contract job for company B. Should I fill in A or B in Employment history? I'm thinking A should be the answer, but not 100% sure.

What employer address should you use? The headquarter of yoru consulting firm may be thousands of miles away from where you actually work.
