Which biographical sketch form to use for NIW EB2

Hello

I just got my NIW EB2 approved and i am trying to send in the 485. Does anyone know which biographic information form to send. Is is G-325A or G-325B or G-325C?

I found it very helpful when I prepared the AOS package (I-485). You don't need ALL the documents, but it's good to know all the stuff...

To kasina,i sent my stuff to NSC in Feb 2009. I applied as eb2 NIW

16 publications and 6 letters (5 from folks outside my university)

I had mininal citation which is about 22 or so.

I was recently approved for NIW EB2 and i applied concurrently without my family initially although i stated in my initial submission that i will send their application later. I mailed 485 for my family and just saw that INS only cleared the check for my wife's 485. They did not clear the one for my kids yet. Has anyone experienced this before?
Does anyone know how to effect a change of name affidavit?
