Thing is, the civil surgeon I went to asked for the notice, and lost it.
Now this is someone who has the blessing of both USCIS, and some esteemed institute of medecine. So I was more than ready to accept her assurances that she'd return it.
Moral of the story? Don't trust woman doctors.
Anyway, now I'm not sure where to send the requested documents.
My case is being processed by the NBC (MSC), and I remember from quickly glancing at the notice after it arrived, that all requested documents were to be sent back to the Lee's Summit, MO address.
I was able to dredge up this address from the USCIS website:
USCIS—NBC
PO Box 648005
Lee’s Summit, MO 64064
My main question is about the PO Box. It seems the NBC has several boxes. (648003, 4, 5, and 6).
Can any other forumite with a recent RFE from the NBC confirm the address please?
