Where to send EAD Renewal App ?

I applied for my I-485 in October 2001 to Vermont Service Center and got my EAD in Dec 2001. I need to renew it now and when I downloaded the form, there is a box where I need to tell them where I had applied and where I should post my EAD renewal application to.

I live in NJ and applied in the EB3 category.

Also, could someone let me know if these documents are enough or if I need to send any more :

2 photographs
$120 fee
Copy of I-485 receipt
Copy of EAD card (front and back)
Copy of I-94

You got approval from VERMONT Service Center means, you have to write Eastern service center, Vermont will be enough. The document you have mentioned is enough. You have to apply at the earliest because your EAD might be expiring in short period of time. It takes about 4 to 6 weeks to get it extended.
