Where is the "supplemental form to I-693"?

I have a question about the "supplemental form to I-693". I can not find it on USCIS website. Everyone know what or where is the "supplemental form to I-693".

It is the form for your shots. The last time I checked the INS does not release this form to the public. Your doctor is supposed to have a copy.

most doctors have the form and once you are done with the requirments of vaccines, ppd and soforth, he should fill the form, sign it and give it to you

Any civil surgeon designated by USCIS should have the form. You may call 800-375-5283 or request online at http://uscis.gov/graphics/exec/forms/index.asp
Get some extra for your civil surgeon if you like.
