Where can I find a list of doctors?

Hi,
I didn't do a physical exam when I submit I-485 in Oct. 2002. Now's getting closer to the processing time, I'm going to do the exam and have the results ready once CIS or my lawyer asks for it.
Could any of you inform me where to find a list of doctors who do this type of exam? Do I need to talk to my doctor first to get all previous exam results?
