When to do medicals? submit along with I-485 or later

In the past I485 adjustment used to take many years and medical are valid for a year only.

Now a days there are some case approved much faster (less than a year) but don't know if it will keep on going. So in the current situation is it better to do medicals and send along with I-485 or wait some time atleast for I-140 approval.

Medical will cost me less than $200 and my lawyer charges me for each RFE atleast $150/hour and I think for this kind of RFE she will charge me atleast 2 hours labor ($300 +80 mislenaous expenses). That make me wonder what is cheaper and clear way to go???

Medical is sent with I-485. They are extending validity untill the approval. It will not become invalid after a year.
