When to Apply EAD, AP renewal? should i need latest Photographs again.

Hi
i am reading few threads, most of them says better to apply the EAD , AP renewal 3 months before it gets expired..

1. should i need the latest photographs again?..

2. should i send my AP documents to INS when we apply for Renewal...?

3. The 3 months adv time is enough or should i apply some more early time or late ?
