When do you apply an Interim EAD?

My EAD application is approved on time. I have the notice of action, and I kept on tracking it online, but the problem is I don't get the actual card. This happened to me twice. My last application was approved June 2008 but till now I don't have the card yet. Well, the card is not addressed on me...its in my employer. My question is:

When do I apply an Interim EAD?
How long is the waiting period after an EAD has been approved?
Do you think I need to apply for a replacement EAD now?
