What to put in I-90 application

Hello, This is second time I have been issued SB1 (SB1 already issued in the past). In order to file I-90 with USCIS, What should I file for the following

1. Location where you applied for your immigrant visa or adjustment of status.
Option- A. Date of original (First time) adjustment of status
B. Date when First SB1 was issued
C. Date when second SB1 was issued?

Which option to select ?

2. Location where your immigrant visa was issued or USCIS office where you were granted adjustment of status.

Option- A. Date of original (First time) adjustment of status
B. Date when First SB1 was issued
C. Date when second SB1 was issued?

Which option to select?

3. Port-of-Entry where admitted to the United States:
Should I fill latest entry in states on SB1 VISA?

For 1 and 2, aren't they asking for date/location of your original green card application/issuance, not either of your SB1s?

For 3 I don't know because I don't know the context of the question. It's not clear from what you posted if they need first time or latest entry.
