I'm an indian physician doing J-1 waiver in an underserved area. I'm in my second year. My employer filed for labor cert for me and it got approved. I went to him to sign documents for I-140 and he asked to extend the employment contract for additional 6 months on his terms. He won't sign my papers without it. I was shocked and felt betrayed. I just couldn't believe him. I'm sure he will keep on blackmailing me as long as I need paperwork from him. I already have a 4 year contract with him and have to pay heavy fine if I leave him before that. What should I do.
Should I file for NIW but for that will I need a 5-year contract from him. Do I have to file a new I-140 if I leave him after 4 years.
Please advise.

Yes.
* You may submit a 3 year contract and a 2 year self practice plan affidavit to fulfill 5 years of NIW service.
* u need a new I-140 for every change of job on NIW physician in underserved area.
