What is the title of check for AOS fee payment

Hi
I am in Boston. I got a 1st NL from KCC on 5/10. I am sososo exciting! Now, I am sending my DSP-122 to KCC and $375 check to DOS (st. louis).

I have two questions:

1. Should I send the DSP-122 form with any copy of diploma or certificate?
2. What should I put into the title of the $375 check for DOS?

don't send any diploma or any certificates just send dsp 122 + 2 photos 2inch by 2 inch +the bar code you received+ dps 230 if you have any dependence meaning husband , wife , kids if no dependence just send dps 122
for the check i did put in the memo my casanumber dv 20111AF0000AFXXXX FOR YOU YOU SHOULD PUT YOUR REEL NUMBER , don't forget to put a stamped enveloope with your adress

the check should be money order or cashier check i prefer cashier check cause you can write a memo on it from your bank and you can truck like i did
then the chek should be adressed to dos as a benificiary meaning you gonna send to the dos a cashier check in thier name just go to your bank and tell then i want to get a cashier check to departement of state and write in the memo your case number dv 2011.............
