What is I-89 Form?

Few days back I spoke with IIO about my wife 485 application, She said that they have incompleted I 89 form, and after a few days I received a mail from INS sataing that your I-89 form is missing or incomplete, go to local INS office with two photo and fill up the form. But When I ask my lawyer he said that I 89 is not available to public, and it is form fill after 485 is approved,

My wife has not done her FP yet, so how come her application is approved ?? Again I try to contact IIO after a few days, but the lady was very rude and she did not answer anything to me and just said to follow what ever is written in the mail.

I think this is an internal form used by INS. I was told by a IIO that because of system gitches, request for this notice gets sent out if you changed address or something like that related to your I-485. In my case, the IIO told me that I can ignore it since my I-485 is still pending and because it was generated due to a system glitch.:)
