What is 45 Day Rule??

I came to know recently that once the labor is approved, the I-140 should be filed within 45 days.

Is this true for PERM process?

Is this true for the old process (RIR or whatever it was called)?

How does this rule effect the companies which claim to have pre-approved labor from the old process?
