What forms when switching from CP to AOS...

I filed my I-130 for CP, but I know that I want to switch to Adjustment of Status, even though my CP case is complete (long story).

What I'm wondering is, what new forms do I need to fill out? I know I need the I-485, and the G-325A, but is there anything else I need to file? I already did the I-864 when I filed for CP.. do I need to do it again, or will it just be forwarded on when I do the switch? Also, I do not intend to work. Do I still need to fill out the I-765? I can't seem to find a straight answer on this!
 
I-864 is not necessary. Just file the I-485 and associated documents (including I-693 medical and a copy of the I-130 approval), and they will figure it out and cancel the CP.

However, whether they will accept the I-485 depends on what you mean when you said your CP is "complete". Was the CP interview done? I don't know if they will accept switching to AOS if the CP interview was already done.
