What format of Vaccination letter from our doctor back home?

I have all my I-485 papers ready. Now I was going to the Civil Surgeon, when my colleauges advised that if I can get the MMR, DPT record from home, for me and my wife, then they may not need to administer it. As me and my wife are planning a family, it is also said that taking the MMR for the wife, is not recommened at all.

Can anyone let me know what kind of format does that letter be in?

Also does it need the MM/YYYY of each vaccination, or the DD/MM/YYYY, which we will have to fish a lot of records to show the doctor proof?

My Mom says she remmebers that I had MMR at 2 mos old, and all others she knows the months, not the dates. Please help

I need to get the fax from home. But do not know what format. My parents have the records but not legible, and in different language, so can get letter from my doctor

First your full name (first and last name) and then your date of birth (it can be in indian format also) but try to get it in words so that there won\'t be any confusion. then 2 sentenses saying that you have been given the list of following vaccinations as per the schedule. That\'s all. underneath, seal and sign of surgeon. You take that to INS doc\'s here and they will add SUPL-I-693 based on your info.
