what form to use .. please advice

I changed employer months ago but still have the previous employer's visa on my passport (valid until the middle of this year). I need to change this since I'll be going home by the end of this year. My current H1-B is good until early next year.
My questions are:
1.) What form should I use to change visa?
2.) Do I need to apply for AP or is my H1-B good enough to get me back?

You have to send your passport to St. Louis or Washinton for Revalidation. In this Forum you have a seperate section for H1 and there search for the topic on 'Revalidation'.

H-1 is good enough for travelling. You need to apply AP, if you are not on H-1.

Refer http://travel.state.gov/hlop.html to revalidate your H-1. If you have an H-1 that has a validity much beyond your return date, you're fine.
