What does "Detailed Review" on this case mean ?

I-140/I-485 concurrently filed in APRIL 2004

I-140 Approved in DEC 2004

I-485 Still pending.

When the lawyer sent an email inquiry they got the following response
"case is currently undergoing detailed review and please allow time for processing."

Case filed in JUN / JULY 2004 have been approved already.

Any ideas on what situations people have got this response.
