Can some one please post the list of documents required to apply for US passport for a neutralized citizen? I got my citizenship on the basis of 5 yr. of green card. Do I need to submit my birth certificate? What about parents birth date? Does it need to be correct on the application.

See attached DS-11 form and instructions for details of what is needed with your application. Seems like you will need photographs, the natz. cert and fees.
A birth cert seems to be required for applicants born in the US. For some one who obtains citizenship through natz, being born outside the US, the natz. cert is the proof of citizenship.
I would recommend that the birthdates of your parents be accurate on the form. I verified them against the dates listed in my parents passports, so that if for any reason in the future, this information is cross referenced, it is accurate.
I plan to apply for my PP in the next couple of days after my Oath which is scheduled for this coming tuesday.
