What documents to send for passport application?

I just took the oath. I am ready to apply for US passport. What documents do I need to submit with the DS-11 form?

1. Natutalization certificate
2. Birth Certificate

Do I have to mail the original Naturalization certificate? How about the birth certificate, do I even need that?

First time applicant can't apply by mail, go to one of those thousands of passport application accepting places (post offices mostly). http://iafdb.travel.state.gov/
You as a naturalized citizen don't need birth certificate. You need to submit the original natz cert.
 
Get the instructions (available with the form at http://travel.state.gov). They are very clear. You need to bring more documents than those you mail (for example you must show some government issued picture ID (like a drivers license) but you don't need to mail it.

Yes, you have to send your original Natz Cert. You get it back when your passport arrives.
 
Assuming you are applying at a post office, you will show your state id (drivers license), hand over your original natz certificate, application form and check covering the fees. You get your id back immediately, and the rest gets sent off to be processed.

After the appropriate wait, you'll receive your passport and natz cert via mail.
