What Documents needed for FOIA for 140/LC

I heard that we need to feel form G-639 and need 485 receipts for FOIA..

Can someone shade more light on this...

Send only G639

Get it siged by a notary.

Provide as much details as possible. Send it in a day /two. It takes at least 3 months to get the docs back

what docs should i submit? Is 485 receipt/140 approval ok?
I provided all mandatory details of section 6 in the form G-639
