Documents needed for employments base Cases
1. Identification MD Drivers license or Passport
2. Work permits and Advanced Payroll
3. W2 Forms and Last 3 Pay slips
4. Copy of approved 1-40 with all attachments.
5. Copy of ETA-750 Labor Certification Package.
6. Employment Letter
7. I-94
8. Any other document given by INS such as I-20(If you had gone to school in US)
9. Birth, marriage or divorce certificates.
10. Childs birth certificates.

This is the list I made by going through different websites ...
1. Fresh employment letter, stating continued interest in employment and job title (make sure it matches with your labor).
2. Fresh medical reports.
3. Last 3 yrs tax returns / W2s and an extra copy of each.
4. Affidavit of support Form I 134 notarized.
5. Original birth certificate(s).
6. Original marriage certificate.
7. All past H1s/H4s (other visas if applicable) to prove you were never out of status.
8. Last 6 months of paystubs to prove continuous employment.
9. Original I94s, since you last entered in US.
10. Passports
11. Driver licence
12. EADs/AP
13. Educational/Degree certificates- originals
14. Past experience letters - originals
15. Copy of labor clearance / I140 clearance (Not all people have these, but my lawyer went out of his way to give me and I dont think they would expect us to have these !)
16. Past 3 months bank statement - to support I134
17. Apartment lease - to prove residence address / marriage etc (not sure how much helpful this is !)
18. MY KID (no kidding.. to prove my marriage is genuine !!!!!)
19. Immigration style photos
and ofcourse original interview letter ...
I had my interview on 6/9/01, at Newark, and it was approved. (Although the officer just asked for Employment letter, pay stubs and Marriage cert.)
This is the list I made by going through different websites ...
