What constitutes as public asisstance

I have utilised the Planned Parenthood Clinic in Ny on several occasions and also I received an emergency temporary medicaid card and HIP insurance card a while ago and utilised the benefits in addition to the emergency medical condition for which it was issued. I want to adjust status to that of a permanent resident. What do I do for the question that asks if you have ever received public assistance or do you intend to?, found on the I 485 form. Please tell me what to do.
