what are other documents needed when submitting I-485?

Hi, I am doing I-140 petition by myself.
If it\'s approved, what are other documents needed to submit with I-485?
What I know are 1) copy of I-140 approval notice 2) birth certificates.
Does the spouse need a separate I-485 form?
Sorry for these fundamental questions.

Medical exam

For the I-693- medical exam, should we schedule the exam even before knowing the approval of I-140? It always takes me three weeks to get
an appointment with the doctor. Or this can only be done after some notice from INS.
I did physical exam each year, I wonder if the result can be used even it\'s done half a year ago before I-485.

You may take a medical exam even before I-140 approval
because it\'s valid for one year,
but you must go to a doctor being specified by INS.
You can find it by calling 1-800-375-5283,
but that doctor list seems old and not be updated often.
 
recommend a doctor?

Can somebody recommed a doctor in Bay Area?
I am in Mountain View.
I just make a apointment with the doctor by telling him for I-485 purpose? Does he know this issue?

I went to the doctor in Palo Alto in the last December,
but he had already retired.

My friend seemed to go to the following doctor in Sunnyvale:
