Visa Revalidation (Two passports)

I have recently rec my new I-797 .

I have 2 Passports. I took a new passport last year with a photo change and my old passport is valid only for the visa stamped in it. So I guess I have to write the passport details from my new one. RIGHT?
Its already stamped on the old passport saying "Passport cancelled. Valid visas within this passport. "

Do I need to send a cover letter explaining that i am including 2 passports and the reason. Its not specified in requirements to send a cover letter. Do I need to send?


Then there r few questions in D-156 which i am little confused like. I have written answers to few of them below. Just let me know if I am wrong.

26. how long do you intend to stay in US --- (visa expiry date on I-797)
27. What is the purpose of your trip--- N/A
28. Who will pay for your trip--- N/A
29. Have you ever been in US -- Yes
WHEN--- <Starting date of first arrival>
FOR HOW LONG --- Presently staying in US
36. Has anyone ever filed for a immigrant visa on your behalf --YES
WHO- <my wife's name who is on H4>

In D-157
9. List all countries visited last 10 yr
<Do I need to write here the dates of my India visit. I have not visited any other country>

17. list all schools attended
<I have not attended any school in US. So should I give the details of school from India>

In present conditions I don't want to get anything wrong and mess up. So that's the reason asking for so many questions

Please let me know if anything in particular I should take care of

Yes a cover letter is needed. I would even suggest putting a bookmark in the old passport- Please affix the MRV in the new passport.
Answers seem to be right.
Immigrant Visa are :I130 or I140. Your wife could not have applied for your i130 if she is in H4. So if your employer has not filed I140, the answer should be NO to Q36

For Q27: another alternate answer is to work for company X
