Just wanted those who will apply in the future for revalidation to know for what reasons they reject and send form 221(g).

I'm typing down whatever reasons were there on the 221(g) form I got. I hope it helps clear some of your questions and avoiding rejections!

FYI: Mine was rejected because I had put home-country-address in item#15/16. You need to put U.S.address/phone there. I did use a electronic-barcoded-form.

We are unable to process your application for a nonimmigrant visa under section 221(g) of the Immigration and Nationality Act for the following reasons(s).

Please provide the following additional documentation. When you return your package, CLEARLY MARK THE OUTSIDE OF THE ENVELOPE "221(g)". Send your reapplication with this sheet to the address above within one year of the date above. DO NOT SUBMIT ANY ADDITIONAL FEES WITH YOUR VISA APPLICATION RESUBMISSION.

(checkbox)(checked) We do not hold visa applications or files. Please resubmit all supporting documents in one complete package. You must include a new, completed and signed DS-156 with photograph and DS-157(for males between the ages 16-45) when you resubmit the complete application package. You must use the Electronic Visa Application Form DS-156, found at http://travel.state.gov AND PRINT THE THIRD PAGE BARCODE INFORMATION. You must provide a self-addressed stamped envelope or prepaid courier airbill and courier envelope for return of your documents.

(checkbox) You did not enclose the passport with the most recent visa in the same classification as the visa your are applying for.

(checkbox) Your passport will expire in less than six months. We issue visas only in passports valid for at least six months.

(checkbox) Your passport does not contain any pages on which to place a visa. Please obtain a new passport or extra visa pages in your current passport.

(checkbox) The photo you enclosed is unacceptable. See the attached photo requirements sheet. Staple your photograph to the DS-156 in the designated space. DO NOT enclose your photo in a glassine or other envelope.

You must use the Electronic Visa Application Form DS-156, which may be found at travel.state.gov AND PRINT THE THIRD PAGE BARCODE INFORMATION. State your U.S. address in item 15 of the DS-156 and your U.S. home telephone number and U.S. business phone number(if you are the principal alien) in item 16.

(checkbox) E visa applicants: You must submit a recent, comprehensive letter addressed to Visa Office, Department of State, describing in detail the nature and function of the investment and the extent of the principal alien's participation in the investment. The letter must contain a statement of the applicant's equivocal intent to depart the U.S. when E status ends. You must also submit a copy of the investment's most recent financial statement.

(checkbox) I visa applicants: You must submit a recent, comprehensive letter on the business/employer's letterhead, bearing an original signature, identifying the applicant and describing in detail the nature and function of the applicant's position.

(checkbox) L visa applicants: You must submit a recent letter bearing an original signature on the business/employer's letterhead addressed to Visa office, Department of State, describing in detail the applicant's position and nature and function of the business.

(checkbox) O and P visa applicants: You must submit a recent,comprehensive letter bearing an original signature on the business/employer's letterhead addressed to VisaOffice, Department of State, describing in detail the nature and function of the business/employer and identifying the applicant and the nature and function of his or her position. O-2 and P applicants must submit a statement indicating that they have a residence abroad which they have no intention of abandoning.

(checkbox) For visa re-issuance to a dependent child included in a parent's passport, please submit: payment for visa fees(nonimmigrant visa application and reciprocity, if required); passport of parent and child; original or certified copy of I-94; copy of the principal alien's I-797; original or certified copy of the principal alien's I-94; and a completed DS-156 awith standard non-immigrant visa photograph. Male dependent children over age 16 must also complete the DS-157.

(checkbox) For a spouse and/or dependent children applying for visas separately from the principal alien, please submit a copy of the principal alien's I-797 and a ceritified copy of the principal alien's I-94(front and back) in addition to all other requirements.

(checkbox) The Immigration and Nationality Act defines a child as an unmarried person under the age of TWENTY-ONE. Your dependent has reached his or her twenty-first birthday and is no longer entitled to derivative visa status. Your dependent must contact the U.S. Citizenship and immigration services(USCIS) to request a change of visa status consistent with the dependent's primary purposes for remaining in the U.S.

(checkbox) We do not consider revalidation of a valid visa until it is within sixty(60) days of expiring. You may travel with your valid visa and valid I-797 Notice of Action reflecting your current employer.
