visa rejection

My mother's visa got rejected at chennai yesterday. I lost my father in sep 2004. I couldn't go to India because of some problems in my pregnancy. I had a Doctor letter. The officer took a look at it and questioned my mother for 15 minutes(He asked abt my brother who is in India, abt Green card etc). Sometimes he was smiling. Then he asked for the Death certificate. After that he took a look at my mother's passport and said that because the spouse's name was not put as Late so &so in the passport, the visa is rejected(my mother took her passport in Dec 2004). It is weird that even after showing Death certificate and Doctor letter he has such an excuse. Please share your thoughts on this. Is this a valid reason for rejection.
