Visa rejection due to I-94 error

Hi all,
Recently my mother applied for B2 tourist visa at Bombay consulate in Nov 2004, Her visa got rejected.
She has three prior US Visas(B2). The last time she entered USA her I-94 was incorrectly stamped with the entry date of Jan 2003 whereas she had entered in Jan 2004.
The consulate officer told her that she overstayed the 2nd visa meaning she entered US in March left the US in Dec and reentered within 15 days .
However in reality she left US in DEC 2003, got her third visa in India and reentered on Jan 2004(which was incorrectly stamped as Jan 2003) .
This gave the impression that she overstayed her visa. Hence they rejected her. The problem is that we did not correct her I-94 while she was here. Is there any way that this can be rectified now. We have her boarding passes from her last trip which shows she came in Jan 2004 and left in April 2004.
Any info will be very useful.
I was thinking of filling form 1-102, but that is for INS , I dont know what procedure can be dont for the correction with CBP, and if there are forms which I cna fill on her behalf.
Its a very confusing issue and has put me in a deadlock situation.
