Visa and RFE relation?

There are many people got RFE. Is there any relationship between Visa and RFE?
I'm try to figure out if there's any correlation.
Could you guys who got approval or RFE post your original visa status? Such as: H-1, F-1, B-1,F-2,L-1.....

I think RFE's are being issued for the following types of cases, as far as I could observe on this forum:

(1) Used AC21 and changed jobs
(2) Got FP'd more than once, and especially if the two FP's are in different locations
(3) Changed addresses and notified INS
(4) INS whim & fancy. ;-)

Number (4) is hopefully pretty low in percentage terms. I personally have been RFE'd but think it is because of number (3).
