If one is on B-2 when O-1 is applied but O-1 comes through after expiry of B-2, can one go to Canada to get the O-1 stamped on to the passport? I understand the O-1 approval notice will include a valid I-94 - with that one should be issued a canadian visa (just to get the O-1 stamped on to the passport) - is that right guys? or am I mistaken/confused???

I don't believe initial O-1 approval notices include an I-94. Only O-1 extensions do.
