Validity of H1B extension - Question

Gurus:

If one got H1B extesnion for three years using approved I-140 and later his I-485 was not approved then what will be the status of H1B extension?

If I have EAD can I request my employer for three year extension using approved I-140, is there any advantages?

Completed H1 6 years, no H1 extension-

My friend completed 6 years of H1 (by sept 2012) and the employer did not file for a new H1. she could not secure another job, she secured admission from Jan 2013, what are the best available options for her. Her labor was approved and I-140 got approved as well.
