use I766 or I765 to apply for EAD?

I am going to renewal my 3rd EAD. Could anyone tell me which form I should fill, I765 or I766?

I-766 is the actual EAD. EAD is called I-766 and you can look at the back of your EAD and see that it says so in the bottom right corner ... For application for EAD you have to use I-765 only.

Form I-765, I-766 is the EAD card !!!!!!!!
