Usage of EAD and H4 Revalidation at US consulate

After my spouse got her EAD and then using the EAD card for working in US OR NOT working using the EAD card.

Can she go to the US consulate and get her H4 visa revalidated/extended, as My H1 is maintained and i m not using the EAD card.
If she can get her H4 visa stamping, then in the DS-156 visa form, what should we fill for the column "Do u intend to work in US ?" ...

As until now we use to fill it as "NO" for an H4 applicant. But now if she is using the EAD card, should we fill it as "YES", and by doing this will there be issues at the consulate ?

Did anyone face this situation, as it is a common practice to maintain H1 and H4 even if the H4 applicant is working on EAD..

----> No because she will be applying for H4 visa. However, after coming to US, she can apply for SSN and start working in US.
