US passport for kid

I am planning to apply US passport for my kid who is 8yrs old and has indian passport. In his permanent resident card[green card] his last name has not used [last name has LNU], and now i want to change the last name in his US passport application, what is the procedure to do this? I appreciate info regarding this.

You might have to officially change his name first (to add a last name). You can do this at your local county court. I don't think you can get a passport in a particular name unless you have supporting documents for that name (state ID, green card, etc).

Call them to find out what paper work you need to fill out.
