What should I reply on a questions:
1.When do you intend to arrive in the US (provide specific date if known) Before I put " I am currently in US" Now in e-form they only have day/month/ year. Can I leave this question blank?
2. Have you ever been issued a US visa?
I have been issued a US visa three times already (continuation H-1B visa)- before I've printed all contries I've been issued a H-1B visa. Now in e-form I can only put one issuance. Which one - the first one?
I will very much appreciate for your reply!

Any day after your appointment or if you have return ticket to US, put that day.
