As such there is nothing to worry about packet4, as you will have all the information available on website and you dont need the appointment letter also as you can take a print out of your case number and you will be done. regarding medicals when you go to the labs or doctors they will have the forms and produce the same printout which you took from web you will be fine.
and for your initial question where the packet3 will be sent, if you have a covering letter specified where you want to get your p4 they will send to the address on the covering letter. if you havent included a covering letter your attorney will get a copy of p4 and a copy will be mailed to address mentioned in item 10.
