Is it ok to give an approximate date for the query
Date of intended departure
(question 1 in Part 3 in form 131) ?



Will they grant the advanced parole 1 year from the day of intended departure or is it the date from when they issue it?

Yeah -- put any date which is about 45 days into the future. (like if you want to send it today, pick a date after Nov 30)

Your AP will be valid for one year from the date of issue -- you won't be bound by the date that was entered.

Also you don't have to give a reason for multiple entry AP either - just leave it blank
 
Would it matter if you mention your travel plan and dates? As far as I remeber my attorney did not put any information for my travel plan.
