Urgent info needed - 1-134 app detail. please help

Reapplying for rejected visa for my MIL. Need to know if in the application form I-134 they ahve asked if u have previously submitted this application before
do we say yes even though her application was rejected? or leave it as none...please clarify , have to send the papers to india.

WE are applying this application for the same person again after rejection..with a shorter duration..can u please let me know what to fill in this section..
we haven't applied for visa for anyone else..
