URGENT: Important AR11 Clarification Needed

Folks,

In the AR11 form, does the 'Port of Entry' and the 'Date of Entry' refer to the last time one entered the US or the first time one entered the US? I had visited canada on AP and was paroled in to the US the last time around...does that mean my POE is the border town with the Canadian POE?

In the past I have used the last DOE/POE and write it (last) after the date/port.
