Urgent Help!!! Do I need to submit I-94?

This weekend I am planning to go to India. My original I-94 (at port of entry) expired on June, 2000 and I got a H1 extention.

Should I only submit the original I-94 (white card at port of entry) or should I submit both the original I94 and the one that comes along with my I-797 extension (right bottom portion of I-797?
