Urgent: H1B/H4 Extensions Filed Separately and Premium Processing

My H1B is expiring on 9/30/03. I filed extension on 6/2/03 with CSC.
I have to travel to Canada in early October. My questions are:
(1) Do I need to convert my case to premium processing?
(2) If I choose to go premium, how can I make sure my wife's H4 extension, which was filed separately on 9/2/03, also get put into premium queue?
(3) Do I need to wait for the H4 receipt number and then start the H1B premium process?

If u file for H4 along with H1 for premium processing, H1 and H4 will be approved together.
 
Thanks. But I already filed them separately before I decide to go with premium. What can I do to get both H1-B and H4 approved as soon as possible? Does it help if I ask HR to attach the H4 receipt to the premium request?
Anybody did this before?
