URGENT: AP Medical Certificate format for exemption

Hello,

My spouse is in India and his left US to India on Advance Parole. His advance parole is expiring soon before he comes back.

Due to medical reasons, he cannot travel back to US before the AP expiry date.

I need to prepare a Medical Certificate to support this situation to INS and apply for extension of AP with an exemption.

I would really appreciate if anyone can help me in providing the a standard format for Exemption Medical Certificates.

I would also like to know, if the key areas which needs to be covered in the Medical Exemption Certificate.
