on the cover of IV approval notice/packet (to be taken to the PoE) you will see a date for entry. which gives you exactly 6 months to enter US.

Yes the spouse ( I am assuming she is not the primary applicant ) can come anytime within the 6 months period of your visa issuance date . The secondary applicant needs to travel with the primar appilcant or at any time within the 6 months period ( provided the primary applicant has already travelled back)
